You should change the input element to text area element which allows you for a multi line input and then use jquery val method to set the value.

Eg:

in HTML

<textarea id="ta"></textarea>

in javascript using jquery you can use

$("#ta").val("FirstName: Matilda LastName: Serunchwart Phone: 32842034984 SSN: 4328409348 YOS: 1990 Income: 304000 tax:6000 Net Income:310000")

and write this on the btn click event like

$("#validate").on("click",function(){

$("#ta").val("FirstName: Matilda LastName: Serunchwart Phone: 32842034984 SSN: 4328409348 YOS: 1990 Income: 304000 tax:6000 Net Income:310000")

})

where validate is the id of the button "validate form data and calculate tax"
