Your instructions are to use Microsoft Access to develop three data entry computer screens for an EHR. You must use sound user interface principles. Each form should use different types of data entry skills and should build in data quality principles. In other word, you need to use different types of entry, such as free text, radio buttons, check boxes, drop down boxes, and the like. Because all of the screens will be part of the same EHR, they need to use the same colors, have the same look and feel, and so on. Each screen should contain at least 8 data elements. All screens should have at least 1 drop down box with data populated.

What is the purpose of the screen?

The purpose of the screen is to take input of the patients medical and treatment histories. EHR is used to make decisions about a patient’s care. EHR screens contain a patient’s medical history, diagnoses, medications, treatment plans, immunization dates, allergies, radiology images, and laboratory and test results.

Why is each data element important to this screen?

Each screen takes input from the data entry operator. Data elements ( radio buttons, check boxes, drop down boxes, and the like) restrict the operator to input specific data related to the data element. This will help in generating accurate results. For example, if only free text input type is specified for date of birth, one may enter in dd-mm-yyyy OR mm/dd/yy or yyyy-MMM-dd. This will create ambiguity in processing data.

Why is the type of data entry appropriate for the data element?

Well-designed screens are easy to read. With clearly labeled, appropriate data types standardiz length, and other field properties such as patterns, precision, and logical data type. Organized fields will reduce input errors and improve quality of data.
