Question

There are several patients in your hospital with different fluid and electrolyte imbalances. Listed below are descriptions of the patients and questions about them. The questions are based on the questions above and our discussions during the Case Study sessions.

A patient has lost plasma volume due to a hemorrhage. The patient lost a substantial amount of blood (1 L) but the hemorrhage is not life-threatening. Assuming the patient has NOT ingested anything and has NOT had any medical treatment since the hemorrhage occurred, what would most likely be the state of the patient’s cells after this loss of plasma? (Hint: assume no tissue or cell damage occurred due to the hemorrhage) A. very swollen B. very shrunken C. normal sized

A psychiatric patient has “psychogenic polydipsia”. He is constantly drinking large amounts of water to flush monsters out of his body. He has what is called “water intoxication”, a term used in medicine to describe his neurological effects. Because he is drinking water faster than he can excrete the water he has A. hyponatremia B. hypernatremia C. hyperkalemia

A patient has a traumatic crushing injury in which many cells and their membranes have been damaged (and many cells were killed). Due to the cell and cell membrane damage and/or cell death this patient likely has A. hyperkalemia B. hypokalemia

Explanation / Answer

Ans.1 option b. I.e. the patient cells will be very shrunken. Exp. Asthere is huge amount of blood loss so as plasma to the concentration of salts around the cells i.nside the the blood stream is comparatively high which causes shrinkage of cells.

ans. 2. Option a. I.e hyponatrimia

Exp. As the patient is having a lot of water so it causes hemodilution which results in decreasedconcentration of sodium and hence causes hyponatrimia

ans. 3 option a i.e. hyperkalemia

exp. As there are damaged cells leading to lose cell membrane so there will be increased concentration of k as it came out from cells leading to hyperkalemia
