Answer: There are a number of mistakes which Newberg and the steering committee made in the initial development of the protocols. Firstly these protocols were developed mainly by the quality manager and the president. The managers, supervisors and the ground level staff were not involved in its development. There was no communication during the drafting process between the people who drafted these protocols and the ones who had to actually implement it. It was because of this reason that the protocols which were developed were nowhere near the ground realities of the work. This was a major mistake made at the time of documentation stage.
In the initial implementation stage there were three major mistakes which happened. Firstly the managers and the supervisors who were actually responsible for implementation were not trained for the purpose. Secondly there was no proper top to bottom communication during the implementation phase which resulted in the lack of proper understanding about the implementation requirements. Thirdly there was no process of monitoring the progress of the implementation process.
