Question

Part A

A woman, age 36, had suffered from anorexia nervosa for 10 years and was admitted to the hospital for treatment. Her weight was about 60% of the ideal for her height (5 feet, 5 inches, 72.6 lbs). She was fed by gavage (nasogastric feeding) with 3L/day of a nutrient solution that provided 1000Cal/L and had the following composition:

Protein hydrolysate              0.37 moles N

Fat                                        42 g

Carbohydrate                       125 g

Sodium                                 21.7 mmole

Potassium                             32 mmole

Chloride                               28 mmole

Magnesium                           8.3 mmole

Phosphate                             16.1 mmole

Calcium                                15 mmole

Zinc                                      157 mmole

Manganese                           45 mmole

Copper                                 15.7 mmole

Iodine                                   0.6 mmole

Iron                                      161 mmole

Complete vitamin mixture

No complications resulted from the treatment & on the basis of the positive N balance & the positive balances of several other nutrient minerals it was concluded that the nutrients were being used for the synthesis of normal body mass. After 21 days, the patient increased 22 lbs in body weight.

a) What is the protein equivalent of the nutrient fluid? What could be concluded concerning its biological value.

b) What differences might be noted in the nature of the nutrients for gavage feeding as compared to total parenteral feeding (hyperalimentation)

c) In the light of present knowledge what additional component might be added to the diet to ensure optimum nutrition.

Explanation / Answer

a) We have 0.37 moles N protein in diet. It means that 0.37 x 14 = 5.18 gof nitrogen. (Moles = g/MW or g = mol x MW) the value is looking very good to increase possitive nitrogen balance in body. now we can say the biological value of given protein is very good.

Note- Possitive nitrogen balance means increase the nitrogen retention in the body which will increase the amino acid level and protein. This process is for body building. The growth hormones and other body growing molecules are also working possitive nitrogen balance.

b) The Gavage feeding is much batter then the parentral fluid ( no dought parentral no need to absorption and digestion but) because we can use parentral feed as maximum limit but gavage feeding is much batter. It is an natural phenomena. It will more banificial to improve body weight.

c) This solution can be increase at the composition of amino acid/ protein. She has major problem of weight loss. Albumin contain diet can be added. However Megnesium and Zinc also can added to improve nitrigen balance.
