I would choose Option C

Word for Word plagiarism means the original material and student work will be almost exactly the same with every sentence in the original material also present in the copied version. That is not the case here.

Paraphrasing plagiarism means the original content is modified a little bit (mainly grammatical changes and re writing but with same meaning) and the original and student version will be more or less the same without acknowledging the original work.

In the above case, the student has done proper paraphrasing. The student has acknowledged that the first and second line was taken from another source (Source was written in brackets) and also have included a new sentence of his/her own (The third one). So I will not consider this as a plagiarism example.
