The latter was assessed by change in scores for healthy eating index (HEI), a measure of diet quality that assesses conformance to federal dietary guidance. Explanation / Answer

1. The independent variable is the variable named "ChangeHEI" as shown in the diagram and the dependent variable is "Number of pounds lost". Since the weight loss post the bariatric surgery is directly affected by the post surgery dietary habits, so the weight loss is dependent of the dietary habits.

2. Ofcourse both are continuous variable.

3. The research hypothesis should be whether the change in dietary habit affected the weightloss significantly or not. This can be translated in terms of model that whether the "ChangeHEI" is significant or not. This can be done using "Pr>Chisq" column, which shows the p-value which is 0.0010<0.05. Hence we conclude that the post surgery dietary habit is effecting the post srugery weightloss
