Temple Regional Hospital is owned by Community Regional Inc. also known CRI, a large mixture of small and mid-sized hospitals across the U.S. CRI requires every hospital to identify and monitor important quality indicators, also sometimes called key performance indicators (KPIs), that affect patient care and to establish thresholds that should be met for each KPI. Temple chose to collect monthly data on the number of infections after surgery. The data collected is shown below:
Hospital administrators are concerned about whether the high percentages of infections (such as 1.76 % in month 12) are caused by factors other than randomness. They are also concerned about what threshold they should establish for this KPI.
1.) Suppose you are told a recent survey disclosed that of 30 million surgical procedures performed in the U.S. last year, nearly 500,000 (1.7%) resulted in infections. What threshold would you recommend the hospital to establish? What actions, if any should management take?
2.) Suppose you are told that the same survey disclosed a wide variation in infection rates. Some hospitals reported surgical infection rates as high as 8% while others consistently reported rates as low as 0.4%. What threshold would you recommend the hospital establish? What actions, if any, should management take?
Explanation / Answer
1). Threshold value means the value above which we will get outliers or the minimum value required for the sufficient working of the phenomena. In the given question among 30 million surgical procedure only 5,00,000 out of them are infections i.e. about 1.7% among them are infections. So, among one million people only 16,667 people have infections or out of per 100 surgery there are 1.6-1.7 got infected .so we will take 1.7 %threshold value as below these values there is no any possibility of getting infections so 1.7 is the correct value of threshold.
Management of the hospital should incresinthe basic facilities like health condition, hospitality,food surving facility, cleanliness in hospital etc. if these all facilities will take place smoothly then the condition of hospital increase and the chances of infections decreases.
2). The hospital with greater number or we can say the hospital with greater % of variation on infections rate means there the medical profession, facility, cleanliness and other conditions are not up to that mark or not better than the hospital having lower% of infections rate .
In the above given data the average % of infections rate is given by
(55/8035)*100 which will give 0.67% which means out of each 100 surgery,there is less than 1 people got infections.so ,for the above given question the correct value is .67% and the hospital with high % of infections are on alarming rate than the hospital with only 0.4% of infection rate .so for both of them the threshold value should be 0.67%.
