3) Two sources: first is "blood forming stem cells from patient's bone marrow itself", second is "blood forming stem cells from other person".

4) In autologous stem cell transplant, patient gets their own blood forming stem cells while in allogeneic stem cell transplant, the patient receives blood-forming stem cells from another person.
