Question

What is the probable reason for the discrepancy?

What is RhIG? What makes a woman eligible for RhIG and what is the goal of this therapy?

Why was RhIG not given after the delivery of the second child?

How does exchange transfusion treat HDN?

A 71 year old male is evaluated by the preadmission department in preparation for heart surgery scheduled later in the week. No previous blood bank history is on record for the patient A sample is drawn for a type and crossmatch. The technologist obtains the following results. Forward Grouping Rh Grouping Patient Grouping Interpretation | anti-D anti-A anti-B control A1 cells |B cells ABO indeterminant . 2 Rh positive Antibody Screen 21 1 reHuli interpretation screen gel IAT cx:ll negative 1. Why is the ABO result not reported?

Explanation / Answer

The ABO typing is the most important test performedin transfusion pactice today. The single most common cause of transfusion related fatalities is due toa patient being transfused with ABO incompatible blood. The reaction occcur because individuals form potent, naturally occuring antibodies to ABO redcell antigens whic they do not possess.

The additional tests to be completed are weak subgroup of A & B.Because weak A or B subgroups have fewer antigens present on the red blood ceells, very weak or negative reactions may occur with anti-A or anti-Bserum,but will give stronger or positive reactions with anti-A,B serum.
usually Group B individuals lack the A antigen and their serum will agglutinate A cells with their naturally occuring anti-A.Their serum will not agglutinate the reagent B cells.

DISCREPANCY:An ABO discrepancy is a situation in which reactions obtained in theforward type DO NOT match or correlate with reactions obtained in the reverse type.
eg;no agglutination when testing a patient with anti -A and anti -B indicates that a patient is group O.

*RhIG means Rh immunoglobuline.

*RhIG is a medication used to prevent Rh isoimmunization in mothers who are Rh negative and to treat idiopathic thrombocytic purpura in people who are Rh positive. It is often given both during and following pregnancy. It may also be used when Rh negative people are Rh positive blood.

*After second delivery there is no need of giving RhIG to the mother, because the mother who is Rh negative will be given RhIG during and afterfirst delivery, which develops the Rh immunoglobulins and presents in her body.so there is no need after secon delivery.

* EXCHANGE TRANSFUSION AND TREATING HDN

Double volume exchange transfusion is mainly used for the management of hyperbilirubinemia and haemolytic disease of the newborn, when other methods of treament such as early and intensive use of phototherapy have been ineffective.
* it lowers the bilirubin level and reduce the risk of brain damage.
*it removes the infant affected red blood cells and circulating maternal antibodies to reduce cell destruction.
*to correct anaemia and treat any potential for heart failure.
