Daniel is a healthy boy who is currently 4 years and 6 months old. He lives with his parents and 7 year old sister. He goes to preschool for ½ a day and then is cared for by his grandmother in the afternoon. He is being referred to a dietitian because his mother is concerned that he is too skinny and has chronic constipation.
Activity Level: Plays outside daily to include jumping, running and climbing.
Mealtime: Picky eater. Does not like fruits or vegetables. Frequently does not want to sit at the table for meals with his family. Frequently doesn’t like what is served so his mother will give him cold cereal and low fat milk or macaroni and cheese. His main beverages are low fat milk and juice. He doesn’t like water. He eats fast food twice a week.
Diet Recall:
Breakfast: Cold cereal with low fat milk
AM snack at preschool (crackers or granola bar with juice)
Lunch: Varies but is usually Ramen noodles, chicken nuggets or a sandwich. Low fat milk.
Snack: Low fat milk with cookies
Dinner: Varies. Spaghetti, hot dogs, grilled cheese, milk.
Snack: Small bag of chips or cookies with milk.
1. Evaluate his growth pattern using a growth chart for boys. (cdc.gov). How would you explain his growth pattern to his mother.
2. How does his diet recall compare to recommendations for a 4-5 year old.
3. Discuss dietary habits that might be contributing to chronic constipation.
4. Provide suggestions for his parents to improve Daniel’s nutritional intake.
Explanation / Answer
His food intake seriously lacks fresh fruits and vegetables which are rich source of fibres. In the absence of which it causes constipation. Also the fast food intake twice a week does not help him in his growth and development.
