During a recent evaluation for chest pain (including a cardiac catheterization), he was diagnosed with angina pectoris, given a prescription for SL nitroglycerin, told how to use it, and referred to your cardiac rehabilitation program for sessions 3 days a week. B.J.'s wife comes along to help him with healthy lifestyle changes. You take the following nursing history: B.J.'s father died of sudden cardiac death at age 42 and his mother (still living) had a CABG x 4 (quadruple coronary artery bypass graft of 4 vessels) at age 52; his hypertension is controlled with nifedipine (Procardia) 90 mg PO qd, which he has taken regularly; he has a 35 pack/year smoking history; an occasional beer ("a beer every weekend with the football game"); and a dietary history of fried and fast foods. His current weight is 205 at 5'8"; waist/hip ratio of 1.21. His VS are 132/86, 82, 18, 98.4°F.

4. To diagonose hyperlipidemia in a patient you need to do his lipid profile. The lipid profile (also known as lipid panel) includes four laboratory tests to be done on pateient's blood. These four tests are-

a) total cholesterol

b) low-density lipoprotein (LDL) cholesterol

c) high-density lipoprotein (HDL) cholesterol

d) triglycerides

5. There are multiple heart related problems that can be identified in the patient BJ

    a) Angina Pectoris: Patient has been diagonosed with Angina pectoris, which is a medical condition that arises when the heart muscle doesn't get as much blood as it needs.This is a life threating condition that usually happens when one or more of the heart's arteries is narrowed or blocked.

b) Patient BJ has hypertention which currently controlled by nifedipine medication. It has been found that hypertensive has been the primary cause of cardiac deaths over the years around the world.

c) Family history of patient BJ also suggests that cadiac diisease runs in the family, which makes him more prone to heart disease than a normal person.

6. The most important problem with the patient BJ is his dietary habits and smoking.

If you calculate patient's body mass index BMI, [ (BMI = weight in kgs / height in meters squared] it comes out to be 32, which means the patient BJ falls into obese category. Person with obesity has a high risk of cardiac attack.

BJ is a smoker who smokes 35 packs/year. As smoking increases the risk of developing cardiovascular diseases. BJ is prone to having stroke as smoking damages the lining of artery which results in fat build up in them. This condition leads to obstruction in supply of oxygen in the blood.

He must be taught to change his eating habits by cutting down or eliminationg fried and fast food from his diet and replacing it with boiled and fresh vegetables, soups and fruits juices. He must also be advised to quit smoking and include daily workout in his routine. He must also be advised to loose weight by incorporating targeted workouts and diet in his routine.
