Question
H.B. is a 62-year-old male in the rehabilitation center. He is recovering after a spinal cord injury. The night nursing assistant checks H.B.’s blood glucose level at 6:45 AM. The day shift nurse assesses the value and compares it to the sliding scale at 7:25 AM. The day shift nurse then administers his medication at 7:50 AM. H.B.’s breakfast arrives at 8:35 AM.1. The patient voices concern with the nurse that there is such a long time span between the blood glucose check, the insulin administration, and the meal. How would the nurse explain the answer? Use EB resources as rationale for your answer.
2. Describe the concept of sliding scale insulin. Include your resources. H.B. is a 62-year-old male in the rehabilitation center. He is recovering after a spinal cord injury. Explanation / Answer
Random blood gas levels to be checked before meals three times a day,, but blood sugar levels checked in the early morning hours can be falsely high due to somogyi effect..!
Due to constant source of insulin bY administration there could be drop In glucose levels in mindnight .. Following which due to this there is accentuation of blood glucose levels in the early morning hours..!So with time glucose levels would get corrected and correspond as per the condition of the patient
2 .Type 2diabtetes can be treated with oral hypoglycemics but if resistance occurs or not compatible then insulin is the preferred treatment.
Insulin. Can be given as fixed dose insulin in which daily prefixed dose of insulin is administred...But this is risky as insulin dose couldn't correspond to blood glucose levels at all times.
So preferred insulin therapy is by sliding scale in which randomn blood glucose checked prior to a meal and a chart is maintained showing insulin dosage which corresponds to blood glucose levels ..; so this provides accurate administration of dose as per glucose. So no problems with hyper and hypoglycemia would occur
