1a)Well in my opinion mission of an Engineer should be clear,that is to focus on precise , genuine and honest work .So that failure would not generate.

2b). Failures can be of many kinds like machine failure,natural calamity, poor assistance by leader, risk involving buisness like share market, Mission Failure here can be any of them because all are equally responsible for failure.
