A. Case 1b: Safari Expedition Steve and his wife Stacey decided to celebrate their 10th wedding anniversary by traveling to Africa to go on Safari. Their tour guide, told them about a waterfall that was off the beaten path in an uninhabited part of Africa. It would take about an hour to get there. Intrigued, Steve and Stacey decided since there were here, they might as well experience everything they could. However, that morning prior to departing on the safari, Steve forgot to put on bug repellent. The following day, Steve noticed a bite mark on his arm. It was a little sore and contained some dead tissue. A few days later, they departed for the United States. Once home, Steve became ill with headaches and a fever. He decided to go to the hospital where you work.

Answer 3 On the basis of symptoms like fever, headache anf then sleeping disturbance, Steve is supposed to be suffering from Trypanosomiasis. The pathogen of this disease is Trypanosoma brucei.

Answer 4 Bug repellents are meant to repel the insects from biting. The bug repellent might have prohibited this illness because the pathogen spreads to humans through the vector i.e. tsetse fly. The pathogen completes its lifecycle in two hosts viz. tsetse fly and humans. The repellent would have prevented the fly to bite and hence the illness woud have been prevented.

Answer 5 The disease has symptoms in two stages

WHO recomendation for disease treatment are pentamidine, suramin, melarsoprol and eflornithine. Pentamidine and suramin are for stage 1.

The disease in patient Steve has already entenered Stage 2 and is to be given drugs which can cross the blood brain barrier.
