answer : 1) Patient centered care:

Patient centerd care is a conceptual health care practice that strives to maximise safety,value ,comfort and support .it includes listening to ,informing and involving patients in their care.

example: when i was working as a staff nurse in superspeciality hospital , in neuro surgery intensive cate unit ,i found ommne patient with spondylosis and he is getting severe pain at his back .

- when i was go to his bed ,he asked me so many doughts regarding the spondylosis ,causes ,symptoms and all it takes about one hour conversation between me and the patient

- i listened everything what he is asking and clarified his doughts clearly and given health education regarding pain management such as taking of rest ,positioning and performing yoga how the symptoms are relieved .

- next day i asked him ,how is your health ,he replied that after doing of yoga my back pain was reduced,and he said feeling comfortable.

- he understood very well and at the last he thanked ma and said to me that ,iam satisfied .

2) the second model is suitable for my example ,that is partnership model .

Why : in partnership model both the health care provider and patient have the choices to health care and in decision making .

- in my example : patient is the source of information and he have the decision making power whether he want to do the activity or not

- both health care provider ,patient both are having their vetos .

3) yes i agree that the administrator play prominant role in the health care delivery team,they have responsibility do the various functions in the hospital ,in various departments ,they need to work with the clinicians to provide effective care to the patients

- the clinician can perform the both administrative role as well as health care provider role .

- clinician have the role in the administration team to provide effective and quality of care to patient and family members
