Question

write ICD 10 CM Codes for this scenario

Description of Procedure: After informed consent was obtained from the parents, the patient was taken to the operating room. General anesthesia was induced. The patient's head was turned to the left side and the posterior scalp was prepped and draped in usual fashion. Sutures from previous surgery were removed A new bur hole site was identified posterior and lateral to the previous entry site to approach the lateral ventricle. Cautery dissection was carried out to establish the subgaleal plane and a pocket was created. The previous incision was opened and the shunt valve was encountered and dismantled. The shunt was found to be plugged Attempts were made to connect a new valve system from the new pocket to the old pocket. This was not successful therefore the entire old shunt system was removed. The abdominal incision was again explored before removing the old shunt. A new valve with a new distal catheter was tunneled from the new pocket. The catheter was guided into the frontal horn and connected to the distal system and the distal catheter was then placed into the peritoneal cavity without difficulty. All wounds were copiously irrigated. The skull defect at the previous site was filled with cranioplasty material and all wounds were sutured closed. Sterile dressings were applied and the patient was taken to the recovery room in good condition Selected Answer:
