The process of coding any case starts with the physician's notes or what is referred to as the SOAP (subjective, objective, assessment and plan). In coding any case we must read through the notes completely, identify key words and phrases, and make note of anything that may be missing, incomplete or confusing so we can query the physician. To code an oncology cases we code the diagnoses, signs and/or symptoms documented as relevant to the case, code the procedures, services and treatments provided at this visit and link every procedure code to at least one diagnosis code. In the end we must double check our work and back code to ensure that each and every code description matches what the physician wrote in the notes, with this we are also ensuring that we have no typos. Understanding the different classifications of neoplasms and the stages at which they are diagnosed can sometimes be a confusing process. There are different ways that neoplasms can be coded which are benign and malignant and understanding these we can code the case in a consistent manner as it is critical for all aspects of the case. When coding an oncology case we must know the anatomical site and behavior of the neoplasm and we can find these in out ICD-10-CM book under the Neoplasms. Understanding the physicians notes and using these guidelines are the efficient way of coding an oncology case which include anatomical location, behavior or cell type, metastatic sites, related conditions, treatments and complications. When coding we must also take into account that not all things are related to the neoplasms so we must look for key words such as due to, secondary to and caused by. By using the general coding guidelines and chapter specific guidelines and applying these to the case will help with assigning the correct codes to the case. These steps are broken down into different categories which include patient evaluation, preparing for treatment, medical radiation physics, dosimetry, treatment devices and special services, radiation treatment delivery, radiation treatment management and follow up care management.
