Case Study 20Microbibloc A 28 year old veterinary assistant goes to the emergency room with fever, cough, and swelling of axially lymph nodes. She also had a small, ulcerated lesion on her lower left wrist. The attending health care provider questioned her occupation. It was revealed that she handles many small and large animals at the clinic. Sometimes, wild animals are brought to the clinic, such as squirrels, rabbits, birds for treatment. Explanation / Answer

The name of the bacteria is Yersinia pestis.

Morphology : It is a short, plump, ovoid gram negative bacillus, about 1.5 × 0.7 micrometer in size, with rounded end and convex sides, arranged singly, in short chains or in small groups. In smears stained with Giemsa or methylene blue , it shows bipolar staining ( safety pin appearance) with the two ends densely stained and a clear central area. Pleomorphism is very common and in old cultures, involusion forms are seen - coccoid, club shaped, filamentous and giant forms. Pleomorphism is characteristically enhanced in media containing 3% NaCl. The bacillus is surrounded by a slime layer (envelope or capsule). It is non - motile, non - sporing and non - acid fast.

In bubonic plague, after an incubation period of 3-5 days , the lymph nodes draining the site of the entry of the bacillus become infected. In some cases, the infection remains localised at the site of the flea bite , with only minor constitutional symptoms ( pestis minor). The glands become enlarged and suppurate in most of the cases. The bacilli enter the bloodstream and produce septicemia. Sometimes there are hemorrhages into the skin and mucosa. Disseminated intravascular coagulation is common and may lead to gangrene of the skin and fingers. Case fatality in untreated case may be 30-90%.

Advise : in the prevention of domestic plague general measures such as control of fleas and rodents are of great important . Proper animal handling is necessary , gloves should be worn while handling animal , fluids and animal carcasses should be incinerated properly. Veterenary health care providers should vaccinate them : a whole culture antigen killed vaccine is recommended. A person exposed to a definite risk of infection , whether vaccinated or not , should be given chemoprophylaxis - cotrimoxazole or tetracycline orally for 5 days.
