Human to Human transmission of Brucella melitenesis

Human brucellosis is acquired mainly through contact with infected animal tissues, ingestion of unpasteurized dairy products or infected aerosols. The case of woman diagnosed with proven brucellosis after her husband suffered a relapse of bacteremia with Brucella melitenesis biotype 3 , which was originally acquired abroad by eating goat cheese.

Case presentation-

A 49 yr old married man was admitted to hospital in May 1991 with a four week history of fever to 40 degree celcius , chills and night sweats. He also noticed a dry cough , some non specific lower abdominal dicomfort and weight loss of 3 kg. These symptoms developed after 2 weeks after his return from Syria and Germany. The patient's last meal in Syria included unpasteurized goat cheese. The lab resuls revealed a total leucocyte count of 6.3×109/L with 46% neutrophils ans 48% lymphocytes and mildly elevated liver enzymes with normal bilirubin . The urine culture was negative and the chest X ray showed atelactasis in the right base also gram negative bacteria culture was found in test. The patient was initially treated with ceftriaxone. 2 days later the patient's temperature returned to normal and he was discharged from the hospital on the same oral medication only. The blood isolates were subsequently identified Brucello melitenesis by the physical laboratory.

The patient completed 1 month of treatment amd ws aymptomatic for 7 months , He denied any further travel and any unpasteurized dairy product. Blood cultures were remain positive for B melitenesis biotype 3 . He was given the same treatment as before and he remained well 12 months after finishing of treatment.

- The patient's wife developed high daily fevers 39 to 40 degree celcius , chills and headaches 2 months after his husband reccurence and her blood culures also yielded B melitenesis biotype 3 after 4 days . For the previous 3 yrs she did not travel outside North America. She ate some caviar imported from Iran one week before her illness but it was cultures and found not to have Brucella species. She occasionally eat pasteurized cheese from an Arabic shop on Montreal . She had regular sexual intercourse with her husband over that period and did not use a barrier method of birth control . She was similarly treated with above treatments and did well in subsequent follow up visits.

Discussion

In conclusion, we believe that human to human transmission of B melitenesis occurs. and that sexual transmission probably plays a role .

In addition, given the seriousness of a disease and the presented evidence, it would be prudent to instruct sexually active patients with brucellosis to use a barrier method until therapy is completed.
