For each scenario identify at least three factors that caused a communication breakdown between the health care worker and the patient.

For each scenario identify what could be done to improve the communication in the given scenario. Provide details and examples.

Mrs. Smith is an 89-year-old patient who is bedridden and very hard of hearing. Nurse Nancy came into her room at 8am this morning to provide morning care to Mrs. Smith. Nurse Nancy knocked on the door, walked in, and turned on the sink and began washing her hands. While washing her hands she explained to Mrs. Smith that she was going to assist her with getting dressed. When Nurse Nancy was done washing her hands she turned off the water and went over to Mrs. Smith’s bed. As Nurse Nancy removed the blankets from Mrs. Smith’s body, Mrs. Smith began to cry and shake

Explanation / Answer

In the given case, the patient has a hearing problem, and the nurse explained the purpose of her visit while washing hands. Means, the nurse might not have observed the patient's (Mes. Smith) response, and probably, Mrs. Smith might not have understood/heard what the nurse is telling. Before assisting the patient with getting dressed, the nurse must confirm whether the patient is ready for that or not. Mrs. Smith might have felt that nurse is forcing to get dressed up, or she might be suffering from fever or other health problem, so she started crying.
