Question

J.L is a 28-year-old female patient is admitted to the oncology unit with a diagnosis of suspected Hodgkin’s disease. The diagnosis is confirmed after a biopsy. J.L. is started on treatment, which consists of colony stimulating factors (CSFs) and chemotherapy.

Colony stimulating factors (CSF) are one type of biologic response modifiers (BRMs). What are BRMs and why are they given to patients with cancer?

What are CSFs? How are they useful to patients undergoing cancer treatment?

What would indicate a need for erythropoietin-stimulating agent like epoetin alpha? What are the nursing interventions, side effects, and patient teaching for this medication? What vital sign is the most important to monitor and why?

What would indicate a need for filgrastim? What are the nursing interventions, side effects, and patient teaching for this medication?

After 4 days, J.L’s labs are: hemoglobin 12.8 g/dL and BP is 168/93. What is the explanation for these changes in labs and BP? What are your concerns?

Explanation / Answer

Ques1: what are brmsand why are they given ti patients with chemotherapy?

Ans: any thing which is given adjuvant to chemotherapy to modify tge response rate of a treatment is labelled under biological response modifier.

Ques 2: what are csfs?

Ans: colony stimulating factors are actully granulocytes stimulating factors to accelerate granulocyte production eventually increasing neutrophil counts.

Ques 3: how are they useful to patients undergoing cancer treatment?

Ans: the patients undergoing treatment for cancer are prone to get neutropenia due to haematological toxicity of chemotherapy drugs. Which cause low leucocyte counts. The csfs helps to recover the WBC so that the duration of neutropenia is shorten and hence the risk of developing infection will also decrease and more over it will help to give the chemotherapy on time which is believed to have better outcome if given on actual time

Ques 4: what would indicate a need for erythropoietin stimulating agent like epoetin alpha?

Ans: chemotherapy drugs can cause bone marrow supression affecting all cell lines including erythropoesis. The erythropoetin stimulators helos to increase the erythrocytes production eventually helping in increasing haemoglobulin.
