How to write an appeal, greif and apology letter on the following:

1. On Oct 22, 17 i was taking an exams but have my phone mistaken on the desk. I completed the computerized exams, got a 98% and later the instructor saw the unhidden phone when i have already logged out. She ask for the phone and i handed it to her politely feeling that she is just going to advice the situation. Upon my surprise, she further handed the phone to the dean and i was immediately given a zero. Meaning i failed that class. When i asked the dean she said is the school policy.

2. On Oct 25, 17 again, i which for clinical but was having this same phone in my pocket. After the vital signs, we were sitted waiting for the other group to join us. I tried to look the time when this instructor saw me. She sent me of the clinical and reported me again to the dean. The dean summon a dismissal for me from the course. I now told the dean that, all these phones trouble is because i was monitoring my ailing child who just passed away when i was looking at a time in the hospital. I just saw that message. The dean asked for a proof of my loss and i got it for her. Now i am punished by suspending me 1 year until fall next year to resume classes, and the zero grade on the previous case remain unchanged. Please help me with this 3 different letters on how to put my case.

It is with utmost respect and humility, that I (name), currently pursuing (course name) in (name of university), apologize for the unfortunate incidents that have occurred on Oct 22 and Oct 25, 2017 due to my carelessness. I sincerely hope that you will allow my explanation to somehow dilute the seriousness of the incidents. The incident on Oct 22, during my exams was a reckless mistake, and I completely take full responsibility for it. However, the intent of leaving the phone was not in any way to cheat during examinations, instead it was to keep updating myself on the health condition of my dying child. I now realize that I should have atleast kept the examiner informed on this situation. The 2nd incident on Oct 25th, was the result of the same intention. My child has passed away and I am now deeply depressed. I once again request you to consider my situation at that time and allow me to carry on with the course studies.
