Please provide recommendations with rationales and prescriptions (including duration of therapy) . Would additional information be needed (lifestyle, PMH, ect…)? If so what kind of information? Also please inculde why the therapy you chose is the best course of action for that patient.
A 27 yo male with no significant PMH presents to the ER with knife-like chest pain and is subsequently diagnosed with an bacterial empyema. He is being managed on hydromorphone (Dilaudid) around the clock. He undergoes surgery to drain the empyema and spends 5 days in the hospital. He is discharged on azithromycin (Zithromax) for the infection and hydrocodone with acetaminophen (Vicodin). He returns one day later with nausea, vomiting, sweating and trembling. What do you think is going on? How can you help him?

As the patient has undergone thoracocentesis to drain the fluid and discharged after 5days in hosptal stay the very next day returned to hospital with nausea ,vomiting,sweating and tremblng....I think the patient has got post operativve infection as he was manifested with nausea vomiting ,sweating and trembling or recurrence of infection .i would help him after confirming the cause provison high antbiotics ,checking the site of wound where the thoracocentesis performed , using of aseptic techniques and applicaton of antibiotic cream
