Rationing of Healthcare Services:

Healthcare services can be rationed by governmental rationing or by allowing those who can afford the services to purchase the services. If you were the chairperson of a medical ethics committee faced with the decision concerning which of the following patients would receive a heart transplant, what decision would you make and what justification would you give to the families of each of the patients for making that decision?

Patient 1: A seventy-six-year-old nonsmoker with good pulmonary and renal function is a good candidate for a heart transplant. His life expectancy without the transplant is one month. The patient has the resources to pay cash for the transplant procedure and follow-up care.

Patient 2: A forty-six-year-old unemployed auto worker with a history of alcohol use and smoking suffered a heart attack. He will die within one month if he does not receive a heart transplant. The patient does not have health insurance, nor does he have the resources to pay for the procedure.

Explanation / Answer

As a chairman person of ethics committee it's my responsibility to make a good decision which can fall under the favor of family as well as patient. Unnecessary burden and tension without results means I am not sticking to my profession.

Patient 1.

Here condition is in favor of patient as well as his family. So here I can make my decision yes to the heart transplant. But only one issues is the age of patient. His age is very near to the level of survive or death. In this case I make personnel interview with the key family member and explained them all the norms of treatment including cost of treatment. I also make clarity about the failure of treatment or any major health related issues in this case. Second thing is that I also interrogate with the clients who assist us in heart transplant because they are the key part of our treatment. As a chairperson I also interrogate with the patient whether he want to go with this treatment or not. If he say Yes or No , then it's my duty to explained all the things to his family.

Patient 2

Patient 2 is somewhat complicated but not impossible. As a member of ethics committee , here my duty is to help such patient to save his life because he is with low age including his family depends on him. Here I show the resources from where they can get the money as well as I ll provide address of NGOs who can help them. Second things I personally involved to help them in this treatment by giving various information for finance. I tell them to make appeal in newspaper to gathered the money.
