She lives a sedentary lifestyle and has gained approximately 30 pounds over the last year following a lay off from her job as a factory worker for 25 years. She owns her home and has two children (ages 10 and 12). She was recently hospitalized for symptoms of a stroke (numbness in the hands and arm and consistent headaches). Tests indicated an elevated blood pressure. Following her recent hospital discharge, her doctor referred her to a health program to assist her with making positive lifestyle changes. This program is sponsored through the local clinic and provides patients with two home visits per month. She will continue regular visits to the doctor to monitor her health. Tracy is very concerned about her health and her ability to continue her visits to the doctor given her employment status. 9Explanation / Answer
As in the described case, Tracy can be diagnosed as Hypertensive with obesity and depression (post traumatic - loss of husband and job). Her lack of will to reduce her weight is seen when she has put on 30 pounds in the last years can be attributed to the loss of her husband and job with subsequent setting in of depression. While in the hospital she would have been attended by physicians and neurologists. Follow up and preventive care is important in this case to prevent further such attacks.
A team to handle this case would first be the 1. nurse who will monitor her blood pressure consistently. 2. A dietician/ nutitionalist will also have to be included to give Tracy a meal plan based on her medical condition and encourage Tracy to exercise and reduce weight in a gradual manner.
