Clinical significance is practically important for the treatment effect , whether it has real noticeable effect on daily life. Clinical significance is the ability for treatment or research experiment to help a client or patient so much that they are returned to normal functioning from a dysfunctional state.

2.Ensure that high risk patients keep follow up appointment within seven days of discharge.

3. Provide translators for the patients who do not read or speak English.

4. Confirm the medication plan and reconcile all the discharge medications.
