An ACO is a network of doctors and hospitals that shares financial and medical responsibility for providing coordinated care to patients in hopes of limiting unnecessary spending.
Managed care is a system of health care emphasizing preventative medicine and home treatment.
Managed care is given at home is useful for the chronic ill people who can offord the money at hospitals.

Yes i agree there would be difference in quality of care verses cost, if you pay more money they will get mobile instruments to the home to give quality care otherwise they need to get inadequate treatment with the limited money. At times peoples has to buy things for home personally whereas in hospital all the costly equipment can be used for the better care providing for single cost at the billing.
In ACO people may not acceptable to get treated for a long time so chronic ill patient would prefer home care. and emergency and critical care people has to be in ACO.
ACO is succeeding with quality care and appropriate utilisation of dollars by Robust health IT infrastructure and data analytics capabilities, Adherence to innovative care improvement frameworks and limiting unnecessary spending.
