1.Its only through proper documentation that claiming process and reimbursements can be done in time and precisely.Expensive patient stays and treatment can be denied due to poor documentation as billers,coders or legal professionals dont have that much time to make out meaning from crude writings.As a result they wont pay the hospital for the services it has rendered to the patient resulting in revenue loss.So documentation is very important.

2.yes reimbursements can be claimed by PT S at outpatient settings.But for PTA s reimbursement for services should be done through a licensed PT under whose supervision and care planning the PTA have worked as per Medicare rules in an outpatient setting.The PT have to forward the claim stating his signature and necessary details supporting the claim.

3.In inpatient settings also reimbursements can be claimed as long as medicare finds proper documentation as well as clinical notes highlighting the need of therapists to benefit the patient.

4.Yes a PTA having one year clincal experience attested by licensed therapist(PT) and currently practising with his /her own license and preferably done a clinical instructor course for credentiality can be a clinical instructor.
