Case Study 2
Mr. P is a 76-year-old male with cardiomyopathy and congestive heart failure who has been hospitalized frequently to treat CHF symptoms. He has difficulty maintaining diet restrictions and managing his polypharmacy. He has 4+ pitting edema, moist crackles throughout lung fields, and labored breathing. He has no family other than his wife, who verbalizes sadness over his declining health and over her inability to get out of the house. She is overwhelmed with the stack of medical bills, as Mr. P always took care of the financial issues. Mr. P is despondent and asks why God has not taken him.

Mr P is old person due to which it's not easy way to treat him as easy as others . In patient treatment his age, stress, family and history take largest part for cure , as a drug treatment we can't proceed further.
1. Family history
Family history is the first investigation tools in the treatment of patients because you come to know wether this patient has gentically dia order or it come due to its lifestyle.
In case of Mr P , he is suffered from CHF . CHF is mainly involved due to family history or some disease like diabetes and due to lifestyle issue. So here we check his family history , may his family member have such kind of disease because of that may he suffer so that we can manage his treatment.
2. Time and date of disease
Mr P is old person so 1st by personnel interview we should asked him from which date means tentative you are feeling such type of symptoms. This is very helpful for treatment plan.
3. Previous and current symptoms
Whenever we are planning a treatment we should know the precious and current symptoms of patient so that we come to know we are going with correct medical treatment or not as well as wo can add new way in our treatment and delete some unnecessary things.
4. Assigning of psychologist
It is the most important tools in patient treatment , you can consider this as one of the tools in family education. In Mr P case he and his Misses are old and due to which they come in frustration. His wife only take care of Mr P and she also old one, she is emotionally touch with her husband, she can't see the declining health of her husband because of that both are come under stress. One thing you have to consider here that stree is the evils of all disease, wether you are taking medical treatment but you have stress means no medical treatment can cure you.
Ist things here that we ulter sad environment into happy environment, you give them assurity that rest of world is also affected with this diseases, lot of treatment present in current scenario, don't take worry , you are 100% cure like this if we treat there psychology then you can get more and more outcomes.
His wife is big tools in treatment of Mr P , if she is happy then husband see her face and become happy and recovered fast.
Hence we nominate 1 psychologist there for all these things.
5. Assigning Dietician
Role of Dietician is also important in treatment of cardiomyopathy and CHf, because smoking and alcohol consumption words this disease and on other side high fiber diet is good for this patient. Hence role of Dietician is most important.
6. Lifestyle
You can tell the patient to awake early in morning and do light light exercise , best way you can go for yoga but stay away from hard gym and weight, walking is also helpful for treatment.
7. Family education
Educated both the couple about disease and regular medical treatment including the things by which disease is created and things to avoid like dos and donts. Educated his wife not to forget time to time medication, do not take stress , be happy and assistant your husband for well management.
Use the pictures and video to motivate both the couple and for best outcomes make 2 time call them that is in morning and night for well news of patient.
9. Outcomes
Time to time you must measure the outcomes for future planning and wether he require new treatment or not.
10. Patient compliance
Without patient compliance you can't proceed further in treatment , you manage his polypharmacy in such away that he donot forget any drug to take.
