Question
This is the case:
We have to come up with a prognosis. But before to the conclusions we came up with 2 hypothesis already (shown below) and now we are discriminating one. In this case we are eliminating the second one.
Hypothesis:
1)Patient is possibly suffering the onset of a left middle cerebral artery stroke caused by his chronic hypertension.
2)Ischemic Strokes could be blocking an artery of the brain of the patient.
***HELP ***:
I need a research paper with data (EBM) that confirms why "Ischemic (Trombolic) Stroke" is not what is causing the patient have all those symptoms. Valid information that shows that Hypothesis #2 is not the right one for this case.
THANKS !!!
Explanation / Answer
hii,
MIDDLE CEREBRAL ARTERY STROKE(MCA STROKE) : Middle cerebral artery is the logest artery in the brain which supplies the blood to the frontal ,temporal, parietal, occiput regions.Middle cerebral artery stroke is defined as the the sudden onset of neurological deficit due to brain infraction or ischemia(death of the tissue). STROKE:It results due to death of the brain tissue where there will be inadequate blood supply ultimately leads to ISCHEMIA.
CAUSES :Smoking, diabetes, hypertension, are the main factors which increases the forming of clots in the body.so when the clot travels and logged in the MCA results in stroke.
SYMPTOMS: - if the block is complete on one side of MCA results in complete facial paralysis of one side. -if the block is partially effects only some parts of the face which will not show any symptoms but affectd arm and leg can be seen. -visual loss may be partial or complete and even loss of control ove movent of the eyes. -speech loss
ISCHEMIC THROMBOLIC STROKE: It also occurs due clot which blocks the artery. usually the ischemic thrombotic stroke occurs due to fatty depositions (plaques) inside the artery known as atherosclerosis which narrows the pathway of artery and blocks the blood flow.
CAUSES: family history of stoke, hypertension diabetes, smoking and high cholestrol.
SYMPTOMS: head ache ,dizziness, weakness or paralysis of one side ,sudden severe numbness, visual disturbance or sudden loss of vision, difficulty in walking, slurred speech or inability to talk
As we seen in the scenario the patient is having the symptoms of -loss of vison like curtain fall for few seconds (visual disturbance) - unable to speak but understanding the commands(inablity speech or slurred speech) -face drooping on right side -weakness in arm - bp 165/100
based on scenario symptoms the case diagnosis is ischemic thrombotic stroke
