Case Study: Mary Pearl, age 60, has come into the physician's office complaining of swelling, stiffness, and arthralgia, especially in her elbows, wrists, and hands. A bone scan revealed acute inflammation in multiple joints with damaged articular cartilage, and an erythrocyte sedimentation rate blood test indicated a significant level of acute inflammation in the body.

Ans 1: The likely diagnosis for Mary Pearl would be an acute episode of Rheumatoid Arthritis.

Ans 2: Rheumatoid arthritis is a painful condition of the musculoskeletal system in which synovial membrane presents between the two joints become severely inflamed resulting in thick and stiff synovium. Subsequently, these membranes are invaded by white blood cells and produce destructive chemical substances there.The cartilage along the articular surfaces of the bones gets destroyed and the bone articular capsule and ligaments begin to erode causing restriction of joint movement.

Male and females are equally affected with this condition. It is usually presented with joint pain, swelling of the joint, limping, poly arthritis, loss of range of motion or stiffness.

Ans 3: There is no cure for rheumatoid arthritis. The treatment option may involve exercise and some medical management with NSAIDS, TNF alpha inhibitor, IL-6 inhibitors, JAK inhibitors, immunosuppressants or steroids.

Ans 4: Steroid can be helpful in rheumatoid arthritis as it helps in reducing the inflammation. It also relieves joint pain and stiffness. A low dose daily steroid regimen is usually followed. It also provides sense of well being. It also helps in preventing joint destruction.
